Intervenciones de educación de enfermería domiciliaria dirigidas a cuidadores de recién nacidos: revisión sistemática

El período neonatal concentra aproximadamente 2,3 millones de muertes anuales a nivel mundial, muchas de ellas prevenibles mediante intervenciones oportunas de atención neonatal. El personal de enfermería desempeña un rol estratégico en la educación de cuidadores primarios mediante intervenciones estructuradas en lactancia materna, cuidado del cordón umbilical y estimulación del desarrollo temprano. El objetivo fue analizar las intervenciones de educación de enfermería domiciliaria dirigidas a cuidadores de recién nacidos. Se realizó una revisión sistemática conforme a la declaración PRISMA 2020. La búsqueda bibliográfica se efectuó en PubMed/MEDLINE, Scopus, CINAHL, LILACS, Web of Science y Embase, complementada con Google Scholar. El cribado se gestionó mediante Rayyan y se incluyeron ensayos clínicos aleatorizados y estudios cuasiexperimentales en español, inglés y portugués. La calidad metodológica se evaluó mediante RoB 2 y MINORS. Se incluyeron 14 estudios con 3.247 participantes entre cuidadores y recién nacidos de 10 países. Las intervenciones mostraron resultados favorables en lactancia materna exclusiva (OR: 2,34; IC 95 %: 1,78–3,08), reducción de complicaciones del cordón umbilical (RR: 0,43; IC 95 %: 0,29–0,64) y mejoras en indicadores de neurodesarrollo a corto plazo (DME: 0,52; IC 95 %: 0,31–0,73). La modalidad combinada de educación presencial con seguimiento virtual o telefónico presentó mayor efectividad y adherencia. Las intervenciones educativas de enfermería dirigidas a cuidadores de recién nacidos muestran resultados prometedores en diversas dimensiones del cuidado neonatal; sin embargo, la evidencia presenta limitaciones asociadas a la heterogeneidad metodológica y al número reducido de estudios, por lo que se requieren investigaciones adicionales, especialmente en contextos latinoamericanos. Se recomienda estandarizar protocolos e integrar estas estrategias en la atención primaria.
